Community Hospice & Health Services

Serving the Central Valley since 1979, Community Hospice & Health Services provides compassionate care, comfort and support to terminally ill patients and their families, growing together because every journey matters. Care extends to over 230 patients every day in private homes, skilled nursing facilities and at the sixteen-room Alexander Cohen Hospice House.
